CONFIDENTIALITY

Blue Mountains Laser Clinic believes in total confidentiality for our clients and at no time will your treatments be discussed with any other persons other than between laser therapists at our clinic, total discretion of your treatment and all personal information remains confidential.

PERSONAL INFORMATION POLICY

All personal information collected from clients is kept at Blue Mountains Laser Clinic as treatment history only.

This information will not be given to any company, private or public institution, unless required to do so by law. It is for our use only.

Photographs taken of clients at Blue Mountains Laser Clinic are only ever used as a personal guide for treatment results and kept at Blue Mountains Laser Clinic, unless you give your permission for us to use photos on brochures etc and then only when media release documents have been signed by you, the client.

Clients must inform us of any medications, illness or injury before treatments as some medical conditions or medications can be contraindicated for laser treatments, peels, microdermabrasion & injectable treatments.

PRE and POST TREATMENT INFORMATION

Clients must follow the pre and post treatment information listed below in the consent section as not doing so may have an adverse effect for treatments.

If any problems or negative outcomes are experienced please call or come in to the clinic as soon as possible to discuss with us.

Complications and side effects although rare can happen with any treatments, these will be discussed in depth with you at consultation.

We can not guarantee results of your treatment as all skin types and conditions respond differently to treatment, however we will always give you honest outcome predictions.

CLIENT CONSENT FORM

All clients are required to sign a consent form in Blue Mountains Laser Clinic prior to any treatment being undertaken at Blue Mountains Laser Clinic

A copy of the information you are required to read and sign prior to treatment is provided below for information purposes. You must complete a consent form in the Clinic

• I understand that there may be some degree of discomfort including inflammation (redness), swelling and bruising with having this treatment / procedure
• I understand that there are no guarantees as to the results of this treatment or future treatments due to many variables such as: age, condition/colour of skin/hair, sun damage/exposure, smoking, climate etc.
• I understand that each case is individual and results vary from person to person, or treatment to treatment.
• I have not had any other IPL treatment within 21 days prior to this treatment.
• I understand that I cannot have another treatment within 21 days of this treatment on the same area, whether the treatment is performed at this clinic location or any other clinic location.
• I understand that to achieve maximum results, I will need several treatments.
• I understand this treatment is a cosmetic treatment and that no medical claims are expressed or implied.
• I agree to refrain from sun tanning booths or sun exposure while I am undergoing treatments.
• I understand that although complications are very rare, sometimes an unexpected outcome may occur and that prompt treatment is necessary. In the event of any unexpected outcome, I will immediately contact the Doctor / technician who performed the treatment. Adverse reactions may include: burning, blistering, grazing or changes to skin colour (hyperpigmentation – an increase in colour, Hypopigmentation – loss of colour)
• I understand that some medications may cause me to be photosensitive to light exposure and if treated with IPL, may contribute to adverse reactions. It is my responsibility to check with my doctor if my medication may make me photosensitive prior to commencing IPL treatments.
• I have disclosed to the treatment practitioner all information that has been requested and agree to have this treatment performed on me.
• I acknowledge that I have read and received a Post Procedure Care Sheet and further agree to follow all post procedure care instructions as I am directed.
